An additional decision related to the type of breast implants you want is related to where the implant should be placed. There are three predominant places where a breast implant can be placed.

Sublandular (over the muscle)

Placed above the pectoral muscles but just below the mammary gland.

Provides the least complicated surgery that is associated with less pain and faster recovery.

Helpful for women with ptosis (sag) that want to achieve some lift without requiring a partial or full mastopexy (breast lift).

For women who are bodybuilding it avoids the awkward appearance of implants during flexing.

Higher likelihood of capsular contracture.

Can obscure mammograms and at best will make it difficult.

Increased risk of the implant rippling.

Increased likelihood for future need of breast lift due to the little support it provides for the implant weight.

Subpectoral (under the muscle)

The upper half of the implant is placed under the pectoralis major muscle, while the lower half is not covered by muscle.

Lower risk of capsular contracture.

Lower risk of visible rippling of the breast implant.

For women with ptosis, creates a less natural appearance with the possibility of it looking at two bulges.

Can look unnatural during flexing.

Rippling is still possible on bottom of implant.

Fully submuscular (under the muscle)

The upper half of the implant is placed under the pectoralis major muscle while the lower half of the implant is placed under related non-pectoral muscles.

Lower risk of capsular contracture.

Lower risk of visible rippling of the breast implant.

Least likely to obscure mammograms.

For women with ptosis, creates a less natural appearance with the possibility of it looking at two bulges.

Can look unnatural during flexing.

 


Breast Implant Place of Incision

Another important decision that you will need to make is the place where incision will occur during breast augmentation surgery. There are four predominant places of incision and their pros and cons are listed below:

Most common approach for inserting breast implants.

Provides good visibility and accessibility to the plastic surgeon for sub-glandular and sub-muscular implant insertion.

Scar that is left blends due to the meeting of the darker skin of the nipple and the lighter skin of the breast.

If a second surgery is required, the incision point can be used again.

For some women, this results in decreased nipple sensation.

Higher risk of nipple numbness.

Any imperfection due to its location on the nipple will be obvious.

Can cause difficulty lactating and breastfeeding for women in the future.
